Care Navigator Administrator

We are looking for a part-time (15hrs per week) Care Navigator to join our supportive friendly team.

The ideal candidate will be a great team player, have excellent organisational and communication skills

Main duties of the job

Main duties:

Scanning and processing clinical patient documents

Clinical coding of patient records

Patient Recalls

Reception cover, including answering telephones and dealing with patient enquiries

Maintaining accurate patient records

Supporting the wider practice team as required

General administrative support

White House Surgery is a friendly, supportive and patient focused GP Practice based in Moreton-in-Marsh. We have a population of just over 5000 patients across the North Cotswolds.

We are a member of the North Cotswold Primary Care Network (PCN), and work closely with four neighbouring practices to deliver enhanced services and improve patient care across the local community, including, First Contact Physio, Mental Health Nurse, Clinical Pharmacist, Wellbeing Coach, Womens Health Clinics, Travel clinics and more.

Situated on the border of four counties, our practice benefits from a rural setting while maintaining links with surrounding towns and healthcare partners.

Job responsibilities

  1. Scanning and processing clinical patient documents
  2. Patient Recalls
  3. Clinical coding of patient records
  4. Maintaining accurate patient records
  5. Reception cover, including answering telephones and dealing with patient enquiries
  6. Effectively signpost patients to the appropriate healthcare professional, including external agencies such as Pharmacy First or 111
  7. Supporting the wider practice team as required
  8. General administrative support
  9. Support in the delivery of contracts, enhanced services and other service requirements

Person Specification

Experience

Essential

  • Have excellent organisational and communication skills.
  • Be confident using computer systems.
  • Have good attention to detail.
  • Be able to work independently and as part of a team.

Desirable

  • Experience in a GP practice or healthcare setting is desirable but not essential as training will be provided.

  • Strong organisational skills with the ability to multitask effectively in a fast-paced environment.
  • Excellent communication skills both written and verbal to interact confidently with patients, staff, and external providers.
  • Understands the importance of confidentiality and professionalism

Desirable

  • Has previous administrative or secretarial experience (medical or NHS experience desirable but not essential)
  • Familiarity with medical terminology, healthcare regulations, and confidentiality standards is advantageous.
